CITY COUNCIL, CITY OF SOUTH SAN FRANCISCO, STATE OF CALIFORNIA
A RESOLUTION APPROVING MODIFICATIONS OF ACCIDENT
AND SICKNESS POLICY AND HEDICAL AND HOSPITAL
SERVICE AGREEMENT.
BE IT RESOLVED by the City Council of the City of South
San Francisco that:
1. The acceptance of the rider to Group Accident and
Sickness Policy No. GA-378620A, issued by Travelers Insurance
Company, Hartford, Connecticut, to the City of South San Fran-
cisco, a municipal corporation, dated July 26, 1967, and
2' The acceptance of the amendment to Kaiser Foundation
Health Plan, Inc., Group Medical and Hospital Service Agree-
ment (Group 245), providing for an amendment of Section 2-A-(2),
Eligibility and Enrollment for Family Dependents, and Section
4-B, Rates and Payment,
is hereby made and the 'City Manager is authorized to accept
and execute said modifications for the City of South San
Francisco.

I hereby certify that the foregoing Resolution was
regularly introduced and adopted by the City Council of the
City of South San Francisco at a regular
meeting held on the 5th day of September
