CITY COUNCIL, CITY OF SOUTH SAN FRANCISCO, STATE OF CALIFORNIA
A RESOLUTION AUTHORIZING ACCEPTANCE OF
PROPOSAL MADE BY HOSPITAL SERVICE OF
CALIFORNIA TO PROVIDE HOSPITAL AND
MEDICAL BENEFITS.
BE IT RESOLVED by the City Council of the City of South
San Francisco that the City Manager be and he is hereby
authorized to
1. Accept the proposal made by Hospital Service of
California, 111 Sutter Street, San Francisco, California,
dated December 26, 1967, and signed by Sherman E. Nichols,
Group Coordinator, providing for hospital and medical benefits
with an effective date of June 1, 1968, provided the rates of
$10.76 per subscriber and $17.80 for dependents is guaranteed
for at least one policy year.
2. Negotiate and execute the necessary agreements and
documents to contract for said hospital and medical benefits.
3. Terminate any and all hospital and medical benefit
programs superseded by the program hereinbefore authorized.
