HomeMy WebLinkAboutReso 4823-1968RESOLUTION NO. 4823 CITY COUNCIL, CITY OF SOUTH SAN FRANCISCO, STATE OF CALIFORNIA A RESOLUTION AUTHORIZING ACCEPTANCE OF PROPOSAL MADE BY TRAVELERS INSURANCE COMPANY TO PROVIDE LONG-TERM DISABILITY INSURANCE. BE IT RESOLVED by the City Council of the City of South San Francisco that the City Manager be and he is hereby authorized to 1. Accept the proposal made by Travelers Insurance Company, 550 California Street, San Francisco, California, dated December 26, 1967, and signed by Joseph A. Barbour, Assistant Manager, providing for long-term disability pay- ments to employees with an effective date of July 1, 1968, provided Travelers Insurance Company files a consent to extend the proposed rates from and including July 1, 1968, to one year thereafter. Said proposal provides for thirty days elimination period following sick leave or salary continuation coverage to age sixty-five (65) for sickness and/or accident. 2. Negotiate and execute the necessary agreements and documents to contract for said long-term disability payments. I hereby certify that the foregoing Resolution was regularly introduced and adopted by the City Council of the City of South San Francisco at a meeting held on the lst' day of by the following vote: AYES, COUNCILMEN NOES, " ABSENT, " regular April , 1968.., Patrick E.
