A RESOLUTION AUTHORIZING AMENDMENT
OF GROUP POLICY NO. GA 816020 -
TRAVELERS INSURANCE COMPANY LONG
TERM DI SABI LITY P0 LI CY.
BE IT RESOLVED by the City Council of the City of South
San Francisco that Travelers Insurance Company Long Term
Disability Policy No. GA 816020 be and the same is hereby
amended as set forth in the rider dated May 20, 1971 (effective
July 1, 1971) and the Assistant City Manager be and he is hereby
authorized to execute said acceptance on behalf of the City.
I hereby certify that the foregoing Resolution was
regularly introduced and adopted by the City Council of the
City of South San Francisco at a regular
meeting
held on the 21st day of June , 1971 , by
the following vote:
