A RESOLUTION AUTHORIZING EXECUTION OF
A GRANT AGREEMENT WITH THE SAN FRANCISCO
FOUNDATION FOR DISASTER RELIEF FUNDS
that-
BE IT RESOLVED by the City Council of the City of South San Francisco
1. Execution of Agreement.
Execution of a Grant Agreement between the City of South San Francisco
and the San Francisco Foundation to provide disaster relief funds to assist in
recovering from the January 1982 storm/flood damage is hereby authorized, and
a copy of said Grant Agreement is attached hereto as Exhibit "A."
2. Signatures.
The Mayor and City Manager are hereby authorized to execute said
Grant Agreement on behalf of the City, and the City Clerk attest their signa-
tures thereto.
I hereby certify that the foregoing Resolution was regularly introduced
and adopted by the City Council of the City of South San Francisco at a regular
meeting held on the 4th day of August , 1982, by the following vote:
AYES: Councilmembers Ronald G. Acosta, Mark N. Addiego, Emanuele N. Damonte,
Gus Nicolopulos; and Roberta Cerri Teglia

REPORTS:
The Foundation requires periodic financial and progra:~:~,atic reports from.
the grantee on the use of the f~mds received. Unless othel~ise indicated,
a progress report Ss due before the third grant pa)~ncnt can be processed,
and a final report is due 120 da)'s after tl~e final pa)~ent. 3]~e format
of the final)c/al section of the report sl,ould be consistent with that
of the final budget approved by the Foundation, and should show a comparison
of actual to b'o,lgeted expenditures. If necc'ssa~y, an :~udit may be
required. O~idelines on bow to prepare a grant report are enclosed.
Pa)nnents on the gra~t are contin§c~t on the sul~nission of progress reports
and other conditions as noted above.
5. 3E3NITORING In order to assess the effectiveness of our grants, the Fotmctation may
~ monitor or con&~ct an evaluati~,n of the program funded by this grant,
EVALUATION: which may include visits by representatives of the Fo~mdation to obse~n~e
the grantee's prograph procedures and operations and to discuss the program
with the grantee's personnel.

It is the usual practSce to require .mn evaluation of the grantee's per-
fon~mnce during the first year of a grant before making pa)qnent for any
successive year.

We suggest that the grantee accept responsibility for media coverage o£._
the grant. Any statement about the Fmmdation's policies, interests or
personnel..should be cleared with the Foundation's offices. The

Ail funds not expended ~or the puqooses agreed to by the grantee and the
Foundation must be returned to the Fo~mdation.
