CITY COUNCIL, CITY OF SOUTH SAN FRANCISCO, STATE OF CALIFORNIA
A RESOLUTION ESTABLISHING BENEFITS FOR MEMBERS OF THE CITY COUNCIL
BE IT RESOLVED by the City Council of the City of South San Francisco that:
WHEREAS, the City Council last established benefits for the members of the
City Council by Resolution No. 129-81 on October 21, 1981 effective July 1 1981; and
WHEREAS, the City Council established a policy of setting forth the details of
benefits and associated City payments for City employees in comprehensive forms
during 1983-84; and
WHEREAS, the provisions of Resolution No. 129-81 provide for benefits for
members of the City Council in summary form; and
WHEREAS, it is appropriate that the benefits and associated City payment
provided the members of the City Council be set forth in detail so they can be
administered in a manner consistent with City benefits policies; and
WHEREAS, the City Council has determined that the following benefits and City
payments shall be available to each current member of the City Council effective~
upon July 1, 1981:
I. RETIREMENT BENEFITS
Pursuant to the Public Employment Retirement Law, a member of the City
Council who qualifies for membership in the Public Employees' Retirement System
and who files an election for membership with the System's Board of Administra-
tion shall receive retirement benefits established for Miscellaneous Employees'
2% at age 60 Plan. The City shall pay on behalf of each member of the Council
who has elected for membership with the System the seven percent (7%) of base
salary currently prescribed by the System for member contributions.
II. INSURANCE BENEFITS
A. Health Insurance. Subject to the terms and conditions of the City's
contracts with health insurance carriers, each member of the City'Council shall
be permitted to select health insurance coverage for themselves and their eligible
dependents from one of the following plans:
KAISER FOUNDATION HEALTH PLAN - "S" COVERAGE
BAY PACIFIC HEALTH CARE PLAN
BLUE SHIELD OF CALIFORNIA HEALTH PLAN
The City shall pay the premium costs for eligible members and their dependents
to the insurance carrier for the plan selected by each member. The effective
date of covarage for health insurance shall be the first of the month following
the month of enrollment of the member and the member's dependent(s). Coverage
shall terminate on the last day of the month in which the member Vacates his or
her elective office.
B. Dental Insurance. Subject to the terms and conditions of the City's
Dental Insurance Plan, members of the City Council and their eligible dependents
shall be provided the following dental insurance:
California Dental Service - Progressive Plan
The Dental Insurance Plan shall be provided to members with no premium costs to
the member or their eligible dependents. Coverage shall become effective on the
first day of the month following six (6) full months from the date the member
was sworn into office. Coverage shall terminate on the last day of the month in
which the member vacates his or her elective office.
C. Vision Insurance. Subject to the terms and conditions of the City's
Vision Insurance Plan, members of the City Council and their eligible dependents
shall be provided the following vision insurance:
Vision Service Plan - Plan "B" - $10.00 Deductible
The vision service plan shall be provided to members with no premium costs to the
members or their eligible dependents. Coverage shall become effective on the first
of the month following enrollment and shall terminate on the last day of the month
in which a member vacates his or her elective office.
D. Life and Accidental Death and Dismemberment Insurances. Subject to
the terms and conditions of the City's contract with Standard Insurance Company of
Portland, Oregon, members of the City Council shall be proVided the following life
and accidental death and. dismemberment insurance:
Term Life Insurance with a face value of five thousand dollars ($5,000.00)
Accidental Death and Dismemberment Insurance in the amount of five thousand
dollars ($5,000.00)
The City shall pay the premium costs for the members to Standard Insurance Company
of Portland, Oregon in accordance with the requirements prescribed by the contract
with the carrier for the payment of such premiums. Coverage shall become effective
on the first day of the month following enrollment and shall terminate on the date
the member_~~ vacates his or her elective office.
III. PHYSICAL FITNESS
A. Recreation Facilities and Programs. Each member shall be entitled to
free admission to City recreation facilities. Each member shall be entitled to
free enrollment in up to five (5) recreation classes during a twelve (12) month
period. Member admission to recreation facilities and recreation classes shall be
accomplished in conformance with the rules and regulations established by the
Parks and Recreation Department.
B. Physical Examination. Each member may undergo an annual comprehensive
multiphasic physical examination at City expense.
NOW, THEREFORE, IT IS ORDERED THAT:
These benefits and associated City payments are hereby approved by the City
Council and shall remain in force and effect until modified by the City Council. This
Resolution cancels and supercedes provisions of Resolution 129-81 adopted October 21,
1981, as it pertains to members of the City Council.
I hereby certify that the foregoing Resolution was regularly introduced and
adopted by the City Council of the City of~South San Francisco at a requ]ar
meeting held on the 26th day of September , 1984, by the following vote:
AYES: Councilmembers Mark N. Addiego, Emanuele N. Damonte, Richard A. Haffey,
Gus Nicolopulos; and Roberta Cerri Teglia
