Box 711 (City Hall, 400 Grand Avenue) South San Francisco, CA City Council Resolution: RES 113 -2017 File Number: 17 -847 Enactment Number: RES 113 -2017 RESOLUTION APPROVING THE FIRST AMENDMENT TO A CONSULTING SERVICES AGREEMENT WITH DU -ALL SAFETY EXTENDING THE TERM FOR A SECOND YEAR AND INCREASING THE CONTRACT AMOUNT BY $125,000 FOR A TOTAL AMOUNT NOT TO EXCEED OF $250,000, AND AUTHORIZING THE CITY MANAGER TO EXECUTE THE AGREEMENT. WHEREAS, on February 24, 2016, the City Council approved a consulting services agreement ( "Agreement ") with Du -All to provide safety program consulting services in an amount not to exceed $125,000; and WHEREAS, in order to assist with staff's efforts to provide on -site safety services, including assessment, development and updates to safety programs, policies and procedures, performance of risk assessments and inspections, and provision of on -site safety training, staff has prepared an amendment ( "First Amendment ") to the Agreement to extend the term for a second year and to increase the contract amount by $125,000 for a total not to exceed amount of $250,000, attached hereto and incorporated herein as Exhibit A; and WHEREAS, staff recommends that the City Council approve the First Amendment. NOW THEREFORE BE IT RESOLVED, that the City Council of the City of South San Francisco hereby takes the following actions: Approves the First Amendment to the Agreement extending the term for a second year and increasing the not to exceed amount to $250,000, attached hereto and incorporated herein as Exhibit A. Authorizes the City Manager to execute the First Amendment and to make any revisions, amendments, or modifications, deemed necessary to carry out the intent of this Resolution which do not materially alter or increase the City's obligations thereunder, subject to approval as to form by the City Attorney.
