A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF SOUTH
SAN FRANCISCO AUTHORIZING AMENDMENT OF RATES - KAISER
FOUNDATION HEALTH PLANT INC.~ GROUP 245 .
BE IT RESOLVED by the City Council of the City of
South San Francisco that the rates and payment schedule
set forth in Paragraph 4 of the Group Medical and Hospital
Service Agreement dated January 3, 1961, to take effect
as of March 1, 1961, for Group 245, shall be amended to
provide for the following rates:
Subscriber $ 7.95
Subscriber and one dependent 15.90
Subscriber and two or more dependents 22.70
Members 65 years of age or over an
additiOnal $1.80 per month
