A RESOLUTION AUTHORIZING APPROVAL OF RIDER
NO.5 TO THE TRAVELERS INSURANCE COMPANY GROUP
ACCIDENT AND SICKNESS POLICY NO. GA-378620A
BE IT RESOLVED by the City Council of the City of
South San Francisco that Rider No.5 to The Travelers
Insurance Company Group Accident and Sickness Policy
No. GA-37862OA dated September 20, 1963, be approved and
executed, and the Mayor of the City of South San Francis-
co is hereby authorized to execute said Rider on behalf
of the City.
I hereby certify that the foregoing resolution
was regularly introduced and adopted by the City Council
of the City of South San Francisco at a regular
meeting held on the ~th day of November
19 63, by the following vote:
AYES, COUNCILMEN
Patrick E. Ahem, Emilio Cortesi, Andrew Rocca,
G. J. Rozzi, and Frank Bertucelli.
NOES, None.
